MIPS Measure 128 (eCQM 69v9):  Preventative Care and Screening: Body Mass Index (BMI) Screening and Follow-Up requires that we document the Health Promotion Plan when the patients BMI is too high or too low. 

Please follow the workflow below during their Office Visit so that we can meet this measure:

  1. On the "SOAP" template
  2. Locate the "Vital Signs" panel.  If BMI Outside Normal Range is indicated, proceed to Step 3.
  3. Select the "Health Promotion Plan" active text link

  1. Document a diagnosis
  2. Follow up in one of the following sections:
    1. Diet
    2. Physical Activity
    3. Referrals
  3. Select "Add"
