Upon reviewing the chart for the patient who was not getting credit for the family history on the finalize template I found that the "History Review" was not completed.
When a patient is seen and the family, social or medical history is not updated (nothing is added or changed) it will not give credit on the finalize template unless a selection has been made from the History Review.
Also, keep in mind that the selection made from the History Review also affects what is pulled onto the chart note document. Failure to make a selection will prevent any of the history from showing on the chart note, "detailed document" and "reviewed, updated" will both pull in all of the history that displays in the grids on the History template onto the chart note and the "reviewed, no changes" will notate that the history was "Reviewed, no changes" on the chart note document.
